  • Patient Intake volunteers are in charge of taking the vitals signs of our patients for food pharmacy. After patients have checked in with reception, Patient Intake volunteers guide our patients from the reception desk to a separate room to take their weight and blood pressure. Once done they transition our patients to the food distribution portion of the clinic and relay vitals sign information to the receptionists.

    • Set up intake area: Ensure the weight scale and BP machine are operational in the corridor between the waiting room and exam room. Have a chair available for patients.

    • Conducting intakes: Call patients and guide them to the room with a weight scale and blood pressure machine. Record patients vital signs onto the patient intake sheet.

    • Communication: Provide intake sheets with vitals to reception to be inputted into Athena. Communicate any extremely elevated blood pressure cases to the food pharmacy coordinator.
